For adults with developmental disabilities.
Supported
Living Services. Individually tailored planning ensures that
adults with developmental disabilities receive the services and supports
they need make their own choices about where they live in their community
and where they work. The goal of this program is enhancing the quality
of life through maintaining as much independence and self-determination
as is possible.
Comprehensive
Services. An
increased level of support and care is available for those adults with
developmental and physical disabilities who require supervised assistance
with everyday activities, ranging from personal care to health and safety
management and daily activities. We offer 24 hour services in a variety
of settings.

Group
Residential Services and Supports. These services are
designed to meet the unique needs of those individuals who require
full support for safety, physical or medical reasons. Support
for all daily living needs is provided around the clock in warm
friendly homes. |

Individual
Residential Services and Supports. These services are
personalized to meet each person's unique needs and choices of
supports necessary to attain their preferred community living
arrangement. Individuals live in either a Host Home setting or
their own community apartment or home and are supported to enjoy
typical community living. Health and safety are paramount to services
provided. Each individual has access to 24-hour support, when
needed, as well as staff and/or providers to assist them in their
home, daily and community living needs. |
Supported
Employment. Starpoint in partnership with the Department of Vocational Rehabilitation (DVR) helps our consumers find employment through job interest and skill
assessments, job shadowing and try-out, job placement, job coaching
and job maintenance/advancement. We have many employers, such as Wal-Mart, Pizza Madness, Home Depot, Taco Bell and Howard’s Disposal to name a select few, who have been very successful in integrating people with disabilities
into regular employment positions.
Life Skills Center. Starpoint’s Life Skills Center located at 1401 Oak Creek Grade, Canon City, (719) 276-1111 consists of Life Skills Classes, the Life Enrichment Program and Self Management Accountability and Responsibility Training (SMART). Life Skills Classes offers a faster paced, more transitional cirriculium in activities of daily living such as; money management, nutrition and cooking , healthy relationships and many more. The Life Enrichment Program includes more moderately paced academic studies, physical exercise activities that including weight gain and loss management, personal hygiene, games and activities, arts and crafts, woodworking, and jewelry making. SMART classes include academics, literacy and technology with specialized activities made up of exclusive participants.

Day
Services. Adult Day Services are a component of comprehensive services and supports,
and round out a consumer's daily activities. These activities also provide
job skills training and socialization skills. Recreational outings,
volunteer opportunities, music, dance, art classes, and holiday celebrations
are some of the things we do -- many of which are provided by community
volunteers.
Community-Based
Services. Individuals with disabilities experience their community by participating
in typical activities such as volunteering at local organizations &
participating in clubs and organizations. We provide a support person
to accompany them if needed.
Site-Based
Services. We support individuals in a non-integrated setting
in a variety of habilitative, recreation and leisure activities. Educational,
safety and seasonal themes are explored on a weekly basis throughout
the year.
Locations. Our services for adults with developmental and physical disabilities
are located in Fremont, Chaffee and Custer Counties and the Denver Metro
area.
Starpoint
Case Management Services. A team of Case Managers is employed in a separate department of the
company. Their job is to meet with every client on an annual basis to
design a comprehensive plan for services for the coming year. Adult
Services staff then carry out the plan, while the Case Managers continue
to monitor implementation. Consumers and their parents or advocates
can also request that their Case Managers hold special meetings in order
to solve problems and to adjust the program plan in order to better
meet the needs of the individual.
