Single Entry Point (SEP)

In May 2020 Health Care Policy and Financing selected our agency to administer the Single Entry Point Contract for Long term Care Medicaid services in Fremont and Custer County. This contract oversees several HCBS-Medicaid waivers that support individuals to live as independently as possible in their homes.
